  • shtf medical diagnosis needed.

    Its 3 pm and you have an acute pain in your ribs, and shoulder on the right side at an 8 on the pain scale, feels like broken ribs, but there was no injury sustained.
    Symptoms :
    Severe pain.
    Shortness of breath
    Unable to take deep breath.


    From 3 pm till the following morning at 9 the rib and shoulder pain worsen to the point of sleeplessness 10 on the pain scale. By 9 am the shoulder no longer is hurting, by 10 am breathing is easier not labored but still hurts to take a deep breath. And there is centralized pain of a 2 on the pain scale.
    What would your thoughts or questions or possible diagnosis be.

      After further questioning Subject reported an injury to his ribs a year ago October, he broke 4 ribs # 3 thru 6. Also has emphysema.

        fibromyalgia, and you are allergic to all known pain meds except for dilaudid.

          The pain is responsive to hydrocodone

            I was going to say either pneumo or hemothorax due to ruptured brocheoles or arteries or atrerioles in the lung.

            Another possibility would be inflammation of the diaphragm due to an unknown cause...

            If any internal bleeding or escape of air into the pleural cavity I would say evac ASAP or go find a vet.....

              First thought was a SPT. primary exam shows both lungs functional and clear. Heart sounds good. Pulse is a little rapid but normal due to the level of pain.blood pressure normal.

                  The Mrs has gall blatter issues that present like that....still....better to get it checked out.....do you have access to a medic or doc?

                  May be related to prior breaks....but might not.

                        The clinical name for the episode is costo-transverse sprain.
                        Its a sprain of the ribs ( go figure) can be caused by sleeping in a weird of different position, moving the wrong way, or a violent cough or sneeze can cause this. The treatment is the same as for any sprain.
